Account Opening

  1. Read fund’s prospectus
  2. Fill out an Account Opening Form (AOF).
    1. Individual/Joint/ITF/FAO
         Note for Joint Account: Accomplish Customer Datasheet for every investor/applicant
    2. Institution/Corporation
  3. Attach a photocopy of the following documents:
    1. For Individual, one (1) valid ID. Please see List of Valid IDs
    2. For Joint Account, one (1) valid ID for each individual/applicant
    3. For In-Trust for (ITF) accounts (below 18 years old)
      1. One (1) valid ID; or
      2. Child’s birth certificate
    4. For institution/corporate accounts
      1. Secretary’s Certificate/Board Resolution authorizing investment with FAMI and its mutual funds
      2. Secretary’s Certificate/ Board Resolution on the authorized signatories
      3. Customer Data Sheet (CDS) of each authorized signatory
      4. One (1) valid ID of each authorized signatory
      5. SEC Certificate of Registration
      6. Articles of Incorporation and By-Laws (Corporation Sole for Religious Institutions)
      7. Latest Audited Financial Statement
      8. Latest General Information Sheet (Cooperative Annual Performance Report or CAPR for Cooperatives)
  4. Submit all originally signed documents to,
    1. FAMI Head Office (Paseo de Roxas, Makati)
    2. any FAMI Investors Center (Binondo, Cebu, Davao); or
    3. any Metrobank Branch (Pouch Code #90020; ATTN: CUSTOMER CARE DEPARTMENT)

Note: FAMI Account Number will be sent via email or SMS within one banking day after receipt of complete documentary requirements.

  3. Professional Regulations Commission (PRC) ID
  4. National Bureau of Investigation (NBI) Clearance
  5. Police Clearance
  6. Postal ID
  7. Voter’s ID
  8. Barangay Certification
  9. Government Service Insurance System (GSIS ID)
  10. Social Security System (SSS) Card
  11. Overseas Workers Welfare Administration (OWWA) ID
  12. Overseas Filipino Worker (OFW) ID
  13. Seaman’s Book
  14. Alien Certificate of Registration (ACR)/Immigrant Certification of Registration
  15. Government Office and GOCC ID e.g. Armed Forces of the Philippines (AFP) ID, Home Development Mutual Fund (HDMF) ID
  1. Certification from the National Council for the Welfare of Disabled Persons (NCWDP)
  2. Department of Social Welfare and
    Development (DSWD) Certification
  3. Bureau of Internal Revenue (BIR) Taxpayer’s ID
  4. Firearms License
  5. National ID
  6. Work Permit
  7. Diplomat ID
  8. Philippine Leisure and Retirement Authority (PLRA) ID
  9. Company IDs issued by private entities or institutions registered with or supervised or regulated either by the BSP, SEC or IC.
  10. Philhealth Card
  11. Immigrant Certificate of Registration
  12. Other valid IDs issued by the Government and its instrumentalities e.g. DTI
