INVESTMENT PROCEDURE
Step 1. Get a copy of our PROSPECTUS and study it.

Step 2. Fill out the investment ACCOUNT OPENING FORMS.
- For individual account:
- For institutional account:
Step 3a. For individual account, please submit a PHOTOCOPY of ONE (1) VALID IDENTIFICATION (ID) CARD.
Or Step 3b. For corporate account, please submit a copy of the following:
- Sole Proprietorship: Business Permit and latest audited financial statements.
- Partnership: Articles of Partnership and latest audited financial statements.
- Corporation:
- Articles of Incorporation
- By-laws
- SEC Certificate of Registration
- Secretary’s Certificate/Board Resolution, authorizing the investment in the fund
- Secretary’s Certificate/Board Resolution on the percentage of ownership/capital stock held by non-Filipino shareholders
- Secretary’s Certificate/Board Resolution on the authorized signatories with IDs
- Latest audited financial statements
- Latest General Information Sheet - For In-Trust For (ITF) Accounts:
- One (1) valid Identification (ID) Card
- Copy of their child’s birth certificate (must be below 18 years old)
Step 4. FORMS OF INVESTMENT
1. CHECK payable to the Mutual Fund of your choice:
- First Metro Save & Learn Fixed-Income Fund, Inc.
- First Metro Save & Learn Equity Fund, Inc.
- First Metro Save & Learn Balanced Fund, Inc.
- First Metro Save & Learn Money Market Fund, Inc.
2. CASH
3. DEPOSIT directly to the Mutual Fund’s Metrobank account:

Step 5. Kindly fax or email the accomplished forms and deposit/payment slip to fax no.: (632)816-0467 or email: FAMI@firstmetro.com.ph.
The original copies of these forms should be sent to FAMI immediately afterwards.

Check payment shall be cleared first before the investment is booked.
Note: For those who directly deposit their investment in the mutual fund’s bank account, the deposit slip shall be the evidence of investment pending receipt of the Confirmation Receipt (COR).
